国際委員会 2018 January CLSI報告

CLSI　27 - 30 January 2018 AST meeting報告
（2018年1月27日～2018年1月30日：テキサス州ダラス）

　2018年1月27日～30日に開催されたClinical Laboratory Standards Institute（CLSI）のAntimicrobial Susceptibility Testingミーティングに、日本臨床微生物学会から国際委員の舘田一博 委員長（東邦大学）と柳原克紀 委員（長崎大学）、大楠清文 委員（東京医科大学）が参加した。3日間にわたるプレゼンテーションおよびディスカッションが行われたので、決議事項を中心としてその概要をワーキンググループ別に報告する。なお、今回の会議で決定された事項については2018年7月のASTミーティングまでは最終ではなく、最終決定版はパブリックコメントを受けた上で2018年の7月に公表される予定である。

　会議の前日に恒例となっているCLSI Education Workshop Presentationsが開催された。今回のトピックスはEpidemiological Cutoff Values（ECVs）: Their Development and Useと題して5名の演者が疫学的カットオフ値の意義と今後の活用法についてプレゼンテーションを行った。5名の演者のスライドは以下のアドレスからすべてダウンロードできる。https://clsi.org/education/microbiology/clsi-workshop-presentations/

　会議の冒頭でCLSI UpdateとしてMr. Glen Fineから、CILSは今年に創設50周年を迎えので、お祝いの行事を予定していること、AST、Antifungal、Veterinary関係の会議への参加者が増加しているのでボランティアを募集していること、などが報告された。

Outreach Ad Hoc ワーキンググループ

　CLSIの広報活動として最新トピックスをニュースレターで紹介することが前々回の会議で決定され、今回Volume 3, 2018年1号 Winterが発刊（ PDF No.1）されたことが報告された。

　なお、本号に掲載されているトピックは以下の7項目である。

Methods Application and Interpretation ワーキンググループ

Intrinsic Resistance ワーキンググループ

　自然耐性（IR）の表（Appendix B1）に以下の変更を行うことが検討され、すべて承認された。

  • C. koseri とC. amalonaticus group において、piperacillinのR を削除する。したがって、piperacillin の欄が必要なくなるので将来的に削除となる。
  • Citrobacter amalonaticus group をCitrobacter koseri の欄に加えて、脚注としてCitrobacter amalonaticus group は、C. amalonaticus, C. farmeri, C. sedlakiiが含まれる。ただし、C. rodentium は臨床的に頻度が少ないので含まない。
  • K. oxytoca とK. varicola の両菌種名を K. pneumoniae の欄に加え、Ampicillin とTicarcillinの欄にRと記載する。
  • 脚注として、Raoultella spp.にはR. ornithinolytica, R. terrigena, R. planticola が含まれ、Ampicillin and Ticarcillin.の欄にRと記載する。
  • K.（Enterobacter）aerogenese と E. cloacaeにおいてCefuroximeのRを削除する。

ブレイクポイントワーキンググループ

  • P. aeruginosaに対するciprofloxacinとlevofloxacinのブレイクポイントについて、これまでのciprofloxacin；S/I/R = ≤ 1, 2, ≥ 4 μg/ml, levofloxacin；S/I/R = ≤ 2, 4, ≥ 8 μg/mlから以下の表のように1管ずつ下げることが承認された。

  • M100のQCテーブルでmethicillinとmezlocillinの両薬剤名を削除することが承認された。
  • M100のTable 4A-2 Disk Diffusion QCレンジにおいて、K. pneumoniae ATCC 700603のcefepime/tazobactamの緑色表記（日常のQCにおいて推奨されるQC株）を解除する。すなわち、本薬剤の日常のQCとしてEscherichia coli NCTC 13353株のみを使用する。

Fosfomycin Disk Diffusion ワーキンググループ

  • M100のTable 2A Enterobacteriaceaeのコメント（44）を以下の文章に置き換えることが承認された；“For testing and reporting of E. coli urinary tract isolates only. Testing of Enterobacteriaceae other than E. coli is unreliable and should not be performed”

Methods Development and Standardization ワーキンググループ

　M100のTable 2B-1（P. aeruginosa）, 2B-2（Acinetobacter baumannii）, G（Enterobacteriaceae ECVs）のColistinの欄に以下のコメントを追記することが承認された；“The MCIs of polymyxin B are predicted by testing colistin.”

Azithromycin Disk Diffusion QC ranges: There were disk and media lot variability but still met the M23 guidelines.
